If you are being admitted from a hospital:

  • Contact the hospital Discharge Planner to discuss the decision to continue care at a skilled nursing facilty.

  • Know that there is usually a sense of urgency. You will be asked to act quickly and we know that. We are fully prepared to assist you every step of the way.

If you are coming from home:

  • Call us at 330-372-1960 to schedule a tour of our facility. At that time, we will talk with you to gain a clear understanding of your situation and answer any questions or concerns you may have regarding level of care or financial considerations.
  • We will provide you with medical pre-admission paperwork that must be completed by your  primary care physician.
  • We will also arrange for a Level of Care Screening that is required by the State of Ohio in order to be admitted.
  • We will thoroughly review all of your payment options and guide you through the process of accessing whatever services you may need.

If you are transferring from another skilled nursing facility:

  • Whether you are transferring from an assisted living, another rehab facility or a nursing home, your first step is to call us at 330-372-1960 to arrange for a tour of our facility.
  • We will spend time with you to gain an understanding of the reason for your transfer and to address any specific questions or concerns you may have.
  • We will work directly with the existing facility to ensure an accurate transfer of all medical records.
  • We will work with you and the existing facility to assist with transportation if necessary.
